Upward Transitions, Inc.

Office Hours: Monday – Friday: 8:00 a.m – Noon and 1:00 p.m. – 4:30 p.m. Upward Transitions serves those who are homeless and impoverished by providing case management and stabilizing resources to meet basic social needs. The ultimate is to assist individuals and families to reach their potential with the ability to support themselves and contribute back to their community. Upward Transitions Case Managers provide a variety of services to clients.   Case Managers will meet with clients to assess their needs and develop a plan to help clients become or remain self-sufficient. Upward Transitions currently provides the following services*:    *  Case Management     * Utility and Rent Assistance     * Retrieval of Legal Documents     * State IDs     * Food Pantry     * Non-narcotic Prescriptions for Clients who are Homeless    *  Linkage to other Services     * Work Search Program for Clients who are Homeless (Call for hours of operation) *Anyone seeking services is encouraged to call in advance to insure Upward Transitions is accepting clients for specific programs. No assistance will be provided or guaranteed over the phone based on a referral from another agency.

    Hello, I am going on 9 weeks of not receiving a payment from unemployment and have not been able to make my rent for May and not sure if I will be able to pay for June’s but I am hoping and I won’t be back at work until June 15th. My apt complex posted this link I’m not sure if I can get assistance or if I qualify but I am wanting to check or maybe get pointed into some sort of direction please. Thanks, Crystal
