The Folsom Cordova Community Partnership focuses on meeting the basic needs of families while acting as a gateway to other services. This is a powerful way to reach populations who would normally “fall through the cracks” of a traditional insular social services delivery system. Community Support Services provides one-on-one crisis assessment; community-based advocacy and referrals, resources, as well as short-term and long-term solution focused case management. Hello, I am a 56 year old male and I have been the live-in caregiver for my disabled 85 year old mother the past eight years. Unfortunately, my mother broke her pelvis and now resides at a board and care home. The house I’m currently living in is on the market to be sold to pay for her care. This means I am 56 and homeless without employment. After 8 years of caring for my mom, my health has suffered. I don’t know where to turn and was hoping you could offer some assistance. Thank you.
