Rowan Helping Ministries’ Crisis Assistance Network can assist with utility (electric, water and natural gas) disconnection notices, eviction notices, heating fuel, medication, dental extractions and deposits*. The Crisis Assistance Network also provides food and clothing assistance. ALL CLIENTS MUST provide the following information before Crisis Assistance Network assistance can be considered: PERMISSION LETTER- DATED AND SIGNED BY THE CLIENT FOR EVERY VISIT: 1. CAN and USDA Food 2. Financial assistance- requested by Non-Household Member for the Client. 3. MEDICATIONS: ONLY THE ADULT PATIENT named on the prescription must apply in person. For ALL Household Members: 1. Copy of Social Security Card or any other official document with the Social Security Number listed (tax return, school records, insurance cards, etc. 2. Photo ID 18 years old and older (Driver’s license, Government ID, School ID, etc.) Clients applying for FINANCIAL ASSISTANCE also must provide for ALL Household Members 18+ 1. Proof of Income: a. Current Paycheck stub. Old pay stubs to prove loss of income. b. Disability (SSI) Income Letter for the Current year. c. Retirement/Social Security Income Letter for the Current year. d. Proof of Unemployment Benefits e. Proof of any other income i. Workman’s Compensation ii. Court-Ordered Child Support Monday – Friday: 8AM – 5PM
